In oot, it should be in a room with lizalfos. Kill them and open the chest.
In master quest, it is in the room where you have to pop the bubbles before time runs out. Pop them and open the chest.

i am always doing that. on zelda phantom hour glass and looking for the last gem i missed a switch that was in an hidden cave that was on the place of the main temple that you continually visit that likes to the other island
where is the switch
if the switch was in front of the door then i found it. i just relised how thick i am there are 2 boxes and one of them has to be on top of the switch

Yes. Sort of, you need the items from the water, forest and fire to do the others. Well more or less those three need be cleared first i think.

I do them forest, fire, water. I really loved the water temple and iron boots.
I hated forest. Too many wall/ceiling/floormancers. DX
Fire was fun too. Quake hammer YAY.

is the fire temple the place that i went into when the fairy was next door at the sumit of death mountain
never mind i am cerntly in the forest temple. what i good guess i had going through the woods ok the heavy wight guards was a big problem mostly the one with a bat if you call it a bat all to find that the girl i was looking for could be dead but considering that it is zelda she is probably kidnapped or surrounded

for the order i will do is go through the forest , lake hyrule , death mountain (volcano) and then go in search for the others or go by my sat nag (navi)
